The lethargy is probably due to not feeling great - he may just be suffering from ich and a tumor, but there's a good chance something is going on internally to have weakened him into this state too. (Depending on if you have plants, this can be as easy as slowly raising temps and adding aquarium salt for a few weeks). Ich is very contagious, and whatever else is going on will need controlled treatment anyway, so I would recommend getting him moved to a QT when you can - do be aware the main tank should be treated for ich as well. He does appear to have what could be an ich infection (the white spots on his body) - breeding stars will only appear on the face/operculi and pectoral fins, so while some of the spots may be breeding stars, the ones on the body are not.

Have his eyes always been lopsided or is that a new development? With telescope breeds it can just be a product of genetics, but if it's a sudden change then something is definitely up. If you can't find any in your area, you may have some luck over in r/askvet, but fish-specialty vets are tough to track down. I don't have much to say about that - my assumption is that it's some kind of tumor, but you would need to get a biopsy to know for sure (and to know what kind, if it is one) so I would recommend looking around for an exotic vet that can treat fish. Wow I have never seen anything quite like the lumps - almost looks like he has a wen growing just in that one spot, but I'm sure that isn't it.
